When you are calling to make your first appointment, please have your insurance information. When you come for your initial visit, please arrive 20 minutes prior to your appointment time.
Items to bring to your first visit
- Photo identification
- Health insurance card(s)
- Social Security card
- Proof of income for sliding scale fee schedule discount
- All medications currently taking
- Medical or dental history
- Medical records (especially) if recent hospital admission or emergency room visit
- Foster parent or grandparent: Guardian or custody papers showing custody of child required
- Adult guardian: Power of Attorney

Patient Centered Medical Home
A medical home is a team approach to better serve you and improve your health. Your medical home team will include your health care provider, others who support you, and-- most importantly –you!
YOU are the most important person on the health care team. Patient-centered is a way of saying that you are the focus of your health care.
A dedicated provider along with a highly-qualified health care team will build a relationship with you to meet your health care needs. Within your PCMH, your team will empower you with ways to improve your health.
Your medical home can:
- Help you manage your health care
- Help answer your health questions
- Listen to your concerns
- Work with other medical experts if necessary
- Coordinate your care though additional services
- Encourage you to play an active part in your own health care
